In one case, the user was opening a session on a Phantom Firewire drive that was not formatted using Apple's Drive Setup utility. Reformatting the Firewire drive resolved the issue.

Disk Utility, its in your Apps; Utitilies folder in OS X, i dont know how to do it in OS 9.
