HTML form in dashcode

Hello there!

I'm trying to make a very simple dashboard widget but can't seem to get it going.

I have a simple HTML form with two fields and a submit button that I want turned into a widget. It passes the form values onto my web page for submission.

If anyone could help me I would very much appreciate it!
