Contacts on MacBook lost all info after iOS 5

After iOS 5, on my 3GS, all my contacts on MBP have nothing but name, and all are marked Company! No email, phone, address, notes, b-day, etc. remain on MBP. Only on iPhone and iCloud. Hmmm. How do I get this back to my MBP

I also have many, many identically named groups on iPhone with only one per named group having any entries. Where did these come from? Why can't I edit them?
