Do I have to make a USB or DVD to Fresh Install Lion?

I suggest you search a bit more deeply. If you don't create a USB or DVD with Lion on it, just where do you expect the installation to come from? Do you have a clone? Do you know why you want to do a "fresh" installation?
You can do it from the Recovery partition, but it will take forever even with a decent internet connection.
