Map of area

I have seen something like this, or just dreamt it, and I would like to give it a try but have no idea how. I have an interactive google map in the site already but this is not what I want for another page. What I would like to do is have a static image of a map of my area, similar to the one attached. The map would be tilted back as shown and hopefully some depth-of-field added so it gets a little blurry on the back end (and maybe the very front). I'd like to use a standard google map if possible. I've searched google and google maps but have not found anything. Can someone point me to a tutorial or walk me through the steps?
