While updating my address book (in 10.8.1), I selected "Choose Custom Image" to save a photo to a person's vCard. The only options it presented were "Defaults," "Recents," and "Camera." There was no "Other" option that allowed me to browse the Finder and select a saved photo.
The "Other" option exists in iMessage (e.g., in "Change My Picture"), so this seems like a bug. I can still drag and drop a photo onto a vCard, but that's a clunky way to do things. Hope this gets fixed in 10.8.2.
