Turn off Sound Check, use equalizer settings, or download firmware hacks that uncap a limit Apple imposed.

Why would Apple impose a limit?
I've found that with my Bose QuietComfort2 headphones that the volume is only acceptable. If I really wanted it cranked up, I have to do it externally.

I believe there is a guideline for safety sound level. Excessive exposure to high sound level using in-ear speakers is responsible for much of hearing loss today.
