Two Restart and Shut Down buttons, Voiceover and can't turn it off, black box?

Righty, I suddenly have two Restart and two Shut Down selections (Apple top left), can I change them back?

I also have the voiceover, no idea how I got it or how to turn it off, as it tells me everything I'm doing meaning music is impossible.

Oh, and I have a black box around my selections of things, and randomly across pages too.

Help is muchly appreciated. :)

Sounds like the Universal Access in System Prefs is turned on. Go to it, check off what is on.
