How do I make icons like this?

Hi. I was wondering how to make icons like the one below for my PowerBook G4 with the reflection effect. And the icon below is great! Credit to whoever made it! Please be quick! Thanks!

Just ask icloud how the icon was made.

I usually use photoshop then send it off to iconographer to complete the sizes.

Yeah, I'd use icongrapher, but do you mean how do you actually photoshop it?
