Can anyone please scan a blank Job report for me?

Hello everyone, what i would like is a good old Job-In report in soft copy (on my computer), the type you get when you put something like an iPod into get serviced. if you have a full one could you please clone the details out in photoshop or similar, i need this urgently for a school thing
