Formating usb key

when i insert my usb key it will not mount but it is listed in the disk utility but when i go to format it i get an error saying: disk erase failed with the error - input/output error! any body know how i can resolve this?

try it in another mac and pc and see what you get after you try another port. if all those fail then you probably got a bad usb key. if a pc works format it as fat then put it back in your mac. fat is a mountable fs for mac.
