A friend of mine who lives in the US is coming over to the UK so I have asked him to pick up a macbook for me. I have decided to go for Applecare which I am going to ask him to get when he buys the macbook.

My question is do I get him to register it under his name and change it later to mine or is it the laptop that is registered so are any changes required?

Don't worry about it.

Here is how it can work:

1) You get your friend to buy you the laptop first
2) Once it is back in the UK, you can buy Applecare from an US based eBay seller for less!
3) Get the activation code (yes, it is really just an agreement code or something, forgot the specific nomenclature for it)
4) Activate it in your name

Your friend can do it on your behalf first, but you have to write to Apple to get it changed in writing... which is quite a hassle really...
