Go to an Apple store or any place that sells Apple products and get a rebate form.
Just follow instructions from it. Make copies of every piece of information you're sending.
Good luck

If this is the same rebate, http://www.apple.com/promo/backtoschool/bts_rebate_us_retail.pdf
And seriously, make copies of everything. If it's denied after sending everything in, phone calls fix the problem.

Yes it is the one. I guess a print out of this form and then filled out is good enough.

Just read the instructions at apple.com/promo. Hit Submit a Rebate and go for it. It took two weeks for me to get my Nano rebate and I just got my printer rebate two days ago. I sent them on the 2nd of July. Pretty good.

Can you do the online thing, like with the US promotion? It's a lot easier than mailing in.

Even if you do it online you still have to mail everything in. Except you save time on processing that's all.
Definitely make photocopies of everything for yourself, cut the UPC's out of all the layers of cardboard, include originals on everything and send it out certified signature required (if available in Canada).
