Online Quiz Script needed

Anyone know where I can get a php based script that will allow me to create an online quiz - I want to present questions with multi-choice answers that can be rated ie. Answer A=3, B=2, C=1. After all questions have been answered a score will be calculated and the total score can then be displayed graphically (in the form of a bar-chart or similar) along with the scores of all participants of the quiz. Thanks in advance.

why not just use surveymonkey.com?

You can get 100 responses to a quiz for free. If you need more responses, you'll have to pay.

Alternately, you can do each question as a poll. I like to use Quimble.com for polls.
