Okay, let me try to explain what I want to do.

I import a movie and iTunes shows an "icon" of the movie in the library. It randomly saves a frame of the movie as the representation of the movie. Is there a way I can advance to the frame I want (like the title) and save THAT as the icon?

You can use a jpeg image for the art instead of a framegrab. I find it makes browsing movies faster. It seems like if you use a poster frame, iTunes has to actually look at the movie before it can show you the poster frame. If you use a jpeg, it moves a little smoother. Besides, I think it looks cooler (I use theatrical posters or DVD covers):

To do this, just drag your art to the little "album art" square. Then right-click your movie, get info and go to the "artwork" tab and delete the screencap so the only image remaining is your jpeg.
