Hm, mine came with one. Maybe that's because I got the Legendary Edition.

It should have come with one. I bought the basic version and I'm pretty sure everyone should have received the same little card with the peel off tag to reveal the code. It was one of the main reasons Xbox Live crashed the first few days Halo came out.
