Double Menus In Mac 10.4

Hi,

New to mac and need some help. I'm getting double menu items such as Shut Down and Shut Down... whenI click on the apple icon. Wondering how to correct this. Do I need a particular update or is there a fix. Any help would be appreciated right now.

a simple log out and log back in again usually fixes this.
