keychain inconsistency?

I am trying to use keychain to save web site " user name"and "passwords".

Sometimes it works just fine...displaying both the user name and password but other times I only get the user name and must supply the password.

Why?
 
If you open
Applications>utilities>keychain access
you can look at what exactly it is saving. Sometimes it is good to go the Keychain Access menu and run first aid. Also, double check that keychain is really saving both.
