How do I set a SSH tunnel thru a proxy?

I have to use NTLM as a proxy if i need to use SSH in school
What command should i use in the terminal?
or is there any gui program similar to putty that i can use? (set it once and one click away ever after)

(the proxy address to be used is localhost:5865)
