Command-W not working

Does this happen to anybody else?

If i triple click on folder, whereby the window is not selected when its opened and i have to click it to bring it to the foreground, I am not able to use command-W to close the window.

A glitch?

Also, sometimes i'm not able to send files to the trash from column view using command-delete.

I don't know about the Command-W issue, but I earlier I was sorting through a bunch of files in column mode, and occasionally when I pressed Command-Delete, it asked for my Admin password.
