i'm pretty sure i've done this in the past for CS3, but i can't remember how i did this.

making 1 page a different size from the rest of the documents. anyone know?

As far as I know, this plug-in (http://www.dtptools.com/product.asp?id=pcid) is the only way to do it. It costs about $100.
