This should be a pretty easy question but i'm having troubles finding much information on this.

I already have an account and code which allows me to run my application on the Iphone itself.

There's a video on the site that goes through all the steps needed to do it. As well, there's a full set of instructions in the Program Portal on the site. Look for "Distribution".

if you submit an app to the App Store, will it always be available to everyone in the world?

I want to submit an app, but only want a specific group of people to have access to it. Is this possible through the App Store? Or is Ad-Hoc the only way to go about this?

Not possible. You need to go with ad-hoc distribution which is limited to 100 devices.
