OS X Disk Utility formatted USB memory not showing in Linux or Windows

I have a USB Flash Memory stick from a friend. All was working well, until I decided to try and reformat it.

I used the OS X Disk Utility "erase" to do this and since cannot find the drive in Linux or Windows.

Both register the USB device, but neither mounts the file system.

I have tried various combinations of File Systems in "erase" and "partition" within the Disk Utility. (Erase includes the MS Dos file system, but the partition utility panel options do not).

I need to move files between Mac & Windows.

ANY ADVICE ON HOW TO FORMAT THE USB FLASH MEMORY TO USEABLE ON BOTH AGAIN?

I had a similar problem with my USB flash drive. I formatted it using every format possible in Disk Utility, including FAT 32. Finally, I went to a Windows PC and right-clicked the drive and selected format. It then began working flawlessly between my Mac an PC.

So, in short, format it using a PC.

However:

1) No FAT file system is even an option within the Disk Utility.

2) The drive does not appear in Windows!
The windows machine recognises the insertion.
It is listed under the: Control Panel -> System -> Device Manager
But no drive is mounted in the Explorer.
