GT with 256 MB means that the specific graphics card has 256 MB of dedicated video ram on board.

NVIDIA GeForce 9400M + 9600M means that there are two graphics card inside the computer (MacBook Pro), one (9400) is an integrated graphics card, which shares the video ram with the main memory (to save battery life), and the other (9600) is the graphics card with dedicated video ram, which has its own memory (256 or 512 MB).

In Leopard you have to switch between those two card by logging out and back in.

PS: The + sign is often used to indicate the word and.

PPS: May I know, how old you are?
