Until the heart muscle that is being deprived of blood dies, or blood flow to it is restored.
Of course, if too much of the heart muscle dies it stops... then you won't feel any pain at all.

As I understand, there are varying degrees depending on the blockage.
If you have a hard one, you're done in a few seconds to minutes. Lost some relatives that way. A very mild one may go unnoticed. So it depends.
