It is still wrapped up in plastic as if it was purchased brand new, so it's hard to check without removal. I don't know Verizon's replacement policy at all, but one rep told my girlfriend that all replacements were brand new, and I was told something else! From what I can tell, it looks brand new. The only thing they didn't include was a brand new charger or battery (cover).

Right. The original charger and battery (attached to the cover) are included. The battery cover has slight scratches on it. The original box and manuals are included as well.
