Photoshop index color?

I am trying to reduce the number of custom colors in my doc. But ever since I did it to one... everytime I try to index something else the color table keeps applying the same changes to evey doc. I don't have any default actions set, and don't think there are any presets with this function Does anyone know what is going on?
