Discounted upgrade switch

My girlfriend is up for the discounted upgrade and wants to give it to me. What is the best way to go about it? AT&T, Apple Store, Apple online store?

If I buy it online and they send it on the 19th, does it have to be activated under her name, or can I just activate it myself? If it has to be under her name initially, I should be able to switch it to mine whenever I want right?

it shouldn't matter. you just put your sim in the phone and it should work just fine. I don't think AT&T cares who uses who's upgrades just as long as there was an upgrade available, they'll let you use it.
