What criteria or consideration does apple make for an apps to be ranked in top 100 ? is it reviews, downloads or staff recommendation? is it automated, or handpicked by apple ?

It's based on how many sales you have in a particular time period in a day. You're probably going to need at least 500 sales a day to be close to the top 100.

Is it based entirely on number of sales? I thought a while back it was changed to include revenue to some degree. I'm sure the big boys complained as otherwise apps like the $10 Sims would have a much harder time being number 1.

Sales and rate of change. If your sales from the last measurement is significantly higher/lower than the current measurement, you'll move up/down the ranks much faster.
Is there anything official on how rankings are done? I've heard a whole bunch of stuff from developers who have tried to glean info based on there experiences, as well as people who claim they know how it works.
