I have been having the same issue though usually around the 6th page or higher, also I didn't experience it on my iPhone 3g only the 3gs
The only way I can resolve it is to un-install and re-install using iTunes.

good way to fix it is to sync to itunes then run a restore from backup. all apps will be fine untill you do a hard reset. mines doing the same thing and it's happening to a lot of people. hopefully this issue will be fixed in 3.1

IDK how you expect to fix it, if you're not willing to try suggestions.
In fact I meant to say. I did try the fix and it only lasted for a few days. Now it's back to the wrong icons again.

Oh, okay. You've tried the adding app trick and deleting app trick and the restore was the last thing. You could try a DFU restore though.
