I recently had that folder with the question mark when I tried to start up my MacBook. I reset the PRAM and then started to boot from the OS X installation disc. But now, I can't select a destination/destionation volume (the third step in the setup). Does this signify a hard drive failure?

Any help is much appreciated.

Do you have an external hard drive plugged in too?

I would take it to the Apple Store and have them take a look at it.
