Vote counting app? Need suggestions

Hi all -

I am a city hall reporter in a small town. I am looking for an app that may or may not exist - a form to help me count votes.

Essentially, I am looking for a program that will eventually let me load a form every week so that I can record each individual vote (by tapping a radio buution_ and then export it to a spreadsheet, preferably excel.

As an example

Item Five A
Unanimous
- or -
Mayor yes/no/absent
Councilman 1 yes/no/absent
Councilman 2 yes/no/absent
Councilman 3 yes/no/absent
Councilman 4 yes/no/absent
Councilman 5 yes/no/absent
Councilman 6 yes/no/absent
Councilman 7 yes/no/absent

Item Five B (etc...)

Any help is appreciated.
