Rearrange DVD titles/chapters

I have a dvd that seems to have chapters messed up. It seems that the link between some consecutive chapters are broken, which means that suddenly the movie skips forward or backward depending on its mood.

Is there a way I can rip the DVD and rearrange the chapters so they line up?
