Macbook doesn't save volume settings

Hi
My macbook doesnt save the headphones volume, I've already saw a thread talking about it but they only sugested erasing plist files and reseting pram, done both even re-installed os x and nothing..
When I plug the headphones the volume get's set to 50% and it gets really loud:mad:
