phone not associating saved 'contacts' with received or dialled phone numbers

just synced my new iPhone with outlook through iTunes. All is good, except that phone is not associating saved contacts with the dialed or received phone numbers. In other words, if I receive a call from an already saved number in my contacts list, caller I is not showing the saved contact's name...only the number.
 
I always save my contacts with the 1+area code (in the US) and they show up just fine when they call. Could this be the issue?
