SLow iMAC, not responding applications

Hi,
Hope some one can help.
I'm using Intel iMAC for nearly 2 years and had never any problems since two weeks my mac started to freeze and being very slow. After restart, startup is really slow (around 12 minutes) and system is still very slow afterwards.

I tried to reinstall OS and also tried to upgrade from Leopard to Snow leopard which didn't help to resolve this issue.
I just run Apple hardware test and I've got following result:

" 4MOT/1/40000003:HDD- 1341"

Does anyone know what this means or can somebody give me advice on this?

Sounds like a hard drive issue. Open Disk Utility and verify the disk and repair permissions

I have the same problem!! imac owner for two years, doing some ps work and computer acting slow. After restarting, applications wouldnt respond!
