I got home this afternoon to find a photo had been posted onto my Facebook page from iOS. I've done this many times with photos I've taken but the worrying thing is this.

1. I didn't post the photo to Facebook

2. I didn't even take my photo, it is showing a photo through the front windscreen of a car, it isn't even my car!

3. There is no geotagging data so I can't find out where it was taken.

This could potentially be a huge issue with iOS.

There is no way anyone else could have got hold of my facebook or iOS login details...

I think it's possible someone you know got ahold of your phone physically, and took a picture, posted it on your wall, and erased all evidence to freak you out.

Either that, or it sounds more like a Facebook problem.

My phone does not leave my side!

If it is a Facebook problem why is the photo in my iPhone photostream library?

Did you recently sell a phone with your credentials still on it? From the very limited information you give someone has your iCloud and Facebook logins. There's a lot of ways for that to happen so I wouldn't blame iOS just yet. Have you tried to Find My iPhone from iCloud to see if any other device shows up? If it does send it a wipe.
