I had to readjust the time on my old iPod 4G at least once a month, my 5G has kept the exact time to the second over a period of around two months.
Does it have a better clock or does it connect automatically to a time server ?

It probably connects to an Apple NTP server when it's on Wi-Fi.

Just like car battery. Once the battery is old and about to die. The clock in your device will stop being precise.
