I got this card:

XFX HD 4890 8750M 1GB DUAL DVI TV
Product Code: HD-489A-ZDFL

It isn't working in my MacPro properly. It displays artifacts or reboots the system under BootCamp. After 15 minutes of idle warm up it works OK...

I've send it to XFX for RMA and they've tested it and "NOFAULTFOUND".

How it's possible ?
 
So you flashed it yourself for OS X?

Did you maybe choose an incorrect ROM? I would make sure the ROM matches up exactly with the model, manufacturer, revision, etc. and try re-flashing.
 
Likely a bad card and XFX may not have done a full test. I would just exchange it. As the card heats up, the heat sink is likely expanding/contracting and shifting it's contact with the GPU.

They're going to send ma an HD5770. I hope it's going to be as fast as my current flashed HIS HD4870 512 MB.

It should be. The 5770's and 4870's are dead even as far as speeds, from what I've seen.
