Buying an iPhone directly from Apple, how does it work?

If you buy an iPhone directly from Apple do you have to go to an Apple Store or your provider to set the phone up? If you switch from Verizon to AT&T or vice versa what do you need to do?
 
When you buy it online, they ask for your account info for your carrier. When you get it, it'll be set up for your account and you'll just have to run through the setup process (which is when it activates). You won't need to go to a store or anything.
