In the past, I have used my wife's upgrade to get the latest phone, and passed along my current phone to her. To do so, all we needed to do was switch our SIMs.

Does anyone know how it might work this time? If I use her upgrade and get a phone through ATT, but then need to convert it to my number, how does that work? Do I just buy a new Nano SIM from ATT formatted for my number?

Yep just get a blank Nano Sim at AT&T -- and order a Nano Sim --> Micro Sim adapter online for like $1 that way her Nano Sim will fit in the older iPhone.
 
If you go to an AT&T store, they'll do it all for you. Just tell them that you want to switch the phones, and they'll give you the SIM cards you need to do so. When the 4S came out, they even gave me a few extra SIM cards just in case something went wrong down the line.

really no need to buy an adapter. At&t will give you a new nano sim to put in the iphone 5 with your # and a new mirco sim to put back in your wife's phone for free.
