I’m applying a cold gel pack under it. Should I put it to the left, close to the screen? I’m guessing where the heat is concentrated?
 
It really depends on the model, but it's mostly in the CPU/GPU area - on the Retina models, air is sucked in on the sides (and somewhat the back) and then blown out of the back of the machine. On the Airs and non-Retina Pros, it's exchanged completely from the back - some examples:

I'd avoid that, cold surfaces can lead to condensation. Condensation means moisture. Moisture and electronics don't play too well together.

The non-retina models also pull air in from the side (via the ports). Looking at the clutch cover (black plastic strip below the display), on the left side, is another intake. The air is expelled towards the middle/right side.
