Re: Reason #1 I've had an app rejected because my instructions on how to use the app were unclear (to a completely clueless caveman). So I added lots of redundant words and the app was quickly approved. Once I even had to submit a short video on how to hold an iPhone when running the app.
Re: Reason #2 Don't developers test their apps on as many devices as they can find (friends, family) like crazy before submission? Should be little reason for this rejection.
Re: Reason #3 Apps that download code and apps that ask for personal information (etc.) fall into the category of not complying with the developer agreement. These rules plus their gray areas on the edge have changed over the years.
Re: Reason #4 This reason was surprising to me, as there seem to be lots of apps with awkward, hard to use user or outright ugly user interfaces in the App store. The ones rejected must have been even worse!
