You purchase the T-Mobile one full price. The T-Mobile one is GSM unlocked, so it will work with AT&T
You won't be able to purchase an AT&T off-contract iPhone online, but if you are able to find an Apple Store that has it in stock, you are able to buy it "device only". As for online, the T-Mobile GSM version is Unlocked and meant to work with AT&T as well.
T-mobile is the only one you can buy online. In the store you can full price for any of them.

looks like only the 128gb option is available to me. But the 64gb AT&T option is available. Is it possible to buy off-contract phone in store?

Buy T-Mobile "no contract" phone from Apple Store and it'll work on any GSM
Buy any Verizon "no contract" phone and it'll work with Verizon, AT&T, & T-Mobile but these can't be bought at an Apple Store, only at Best Buy.
Buy any AT&T phone at full retail from Apple Store and it'll be unlocked to any GSM

I don't think the AT&T phones come unlocked. Only the Verizon or T-Mobile.

Can you get a Verizon "device only" in an Apple Store? I was reading a thread where they mentioned AT&T "device only" but there's too much mixed info out there.
You can only buy the T-Mobile full price in the Apple Store. Once again, the T-Mobile FULL PRICE/CONTRACT FREE works for AT&T.
Can you reserve one on Apple.com and then just request to pay full price for it at the store when you pick it up?

I have essentially the same question - How can I purchase a verizon "device only" phone on apple.com for store pickup? It seems like I would need to provide verizon account details, which I don't have.

Not possible to reserve one for in store pickup. You can only reserve and pickup the T-Mobile which will be unlocked for use on any GSM carrier (ie won't work with Verizon)
To buy a Verizon or ATT model for full non contract price you need to Apple Store.
I got mine off apple.com/ you gotta get that version off apple or else it will be locked to T-Mobile - I can put my wife's AT&T sim in my t-moible i6+ and it works flawlessly
