iPhone Bluetooth in Car

I have a Lexus GX470, when I make a call from my iPhone through the bluetooth in car speaker/microphone system I cannot hear the recipients phone ringing. There is no way to tell if I have connected. The person picks up the call but I cannot tell that I have established a call. They have already said hello before I can hear anything. Anybody had this issue?
