Things are duplicated

We have a G5 running 10.4.11 and in the Apple Menu Bar it has the restart, force quit, log out and shut down selection menus duplicated.

I ran Disk first aid from the disk and ran Tech Tools Deluxe with no luck.

What could be causing this?
