I've been using Navicat Premium Essentials with our SQL 2000,2005,2008 and a few MySQL databases with pretty decent luck. Can export to CSV from the UI as well.

Got it through the App Store

+1 for Sequel Pro, I tried Navicat and whilst it does some nice extra things (database syncing for one) I can't really justify the extra cost.

For beginners / intermediates Sequel Pro is pretty amazing.

MySQL Workbench

I've been using MySQL Workbench and it works ok. The only other thing I've used is phpMyAdmin which is good, but MySQL Workbench seems slicker though I've only done basic tasks with both.
