Best note taking and recording them same time - OSX/iOS

Hi guys. Can anyone recommend a great note taking app for mac/iPhone where while writing at the same time you can record them? Also another important future has to be syncing over iCloud across Apple platforms. Thanks in advance.

Circus Ponies Notebooks allow for audio recording that syncs with text, and comes in both mac and iPad versions. I've written about how I use it here. They don't have iCloud integration, but I use DropBox and MacDropAny to keep everything in sync.

This is for OS X only, but MS Word 2011 has a simultaneous recording/note-taking feature (in the Notebook Layout View) and it works pretty well :)
