How do we add contacts to iPhone 5 from simcard?

iPhone doesn't use the SIM card for contacts.

How do we then for example, add contacts from iPhone 4 to iPhone 5? or in other cases like from a Samsung phone to iPhone 4? Is manual entering the only way?

Use iCloud, iTunes or iExplorer to backup your contacts.

If you have a SIM card from another phone that has contacts on them, you can import them onto your iPhone as follows:

Settings > Mail, Contacts, Calendars > scroll down and tap on "Import SIM Contacts" button

I've never been in a position to try this but if you have a SIM with contacts, it sure sounds like that'd do the trick.
