Audio coming out significantly quieter on left audio channel...

Audio coming out significantly quieter on left audio channel through headphones. I have tried two different headphones and I get the same result.

Is this a hardware issue or software?

I would also like to add that when ever I plug in anything that is USB the system freezes for a couple of seconds.

Mid 2011 Macbook Air.

Go to system prefs, select audio, drag balance slider to the center.
